IP Address and Name Service Request
  • This electronic request WILL NOT be processed unless sent by Cal Poly Staff/Faculty. Network Administration will confirm this request via phone. A request sent by Cal Poly Staff/Faculty impiles that person has taken responsiblity for the use of the assigned IP address.
  • Your IP Address / Name Service Request will be processed on the Tuesday or Friday after you submit the form to Network Administration.
  • Contact Network Administration (756-1295) with any questions.
  • Text fields with a bold name are required for the form to be processed.
  • Please enter the full E-mail addresses in the appropriate boxes - i.e. jdoe@calpoly.edu not jdoe
